What is the best way to paint a design onto a car?

I was wondering if anyone has any knowledge of hand painting on cars. I do not want to spray paint. Do I need to use a specific type of paint or would car paint work? Would it hold better if I went over it with a clear car coat afterwards? Should I prepare the existing paint in any way before painting on top of it? Thanks!
